Alcohol Beverage Permit Application (Download Form)

Individuals or groups who consume or serve alcohol at Kemp Station must have an approved Alcohol Beverage Permit. The form should be submitted at least 2 weeks prior to your arrival at Kemp.

Driver Authorization Form (Instructions & Form/s)

If you are already authorized to drive a state vehicle through the UW System or another state agency (e.g., WDNR), you do not need to complete additional paperwork to drive a Kemp Station vehicle.

If you are not a UW System or State of Wisconsin employee, you must supply the following evidence of insurance in lieu of completing the Driver Authorization form:

$1,000,000 auto liability
Non-owned vehicle liability coverage
Comprehensive/collision coverage, up to the value of the vehicle

Please check with your home institution's risk management department for this proof of insurance.

Project Information Request Form (Download Form)

Researchers using Kemp Station must submit an annual Project Information Request Form for each project they are working on while at Kemp.

Release of Liability Form (Download Form)

Users who will have children with them, age 17 or younger, must complete and submit this form prior to their arrival. Adults leading youth groups in organized activities while on Kemp Station are responsible for the safety and supervision of their participants and do not need to complete Release of Liability forms.

Use Record (Download Form)

A use record must be completed before departing Kemp Station. Record each individual who stayed at Kemp, last name and first initial, and the number of nights stayed. The completed form can be left at the Office or at the building used by your group.
